I do not understand the meaning of “main image” in the products. As You know we are selling hand knitting yarn. Nearly with all our products we created Variants (=colours/shades) of these.
For the customer on the overview pages we want to show a teaser image. For my understanding it has to be the main image, no matter which variant is the preselected one. In fact prestashop shows always the preselected variant on the overview pages.
To show my teaser image there I match it with the preselected variant. But when the customer opens the product detail page he sees first the teaser image and has to click on the shade image to see the right shade of the selected variant.

In context of your case it is necessary to distinct two kind of products
- products with combinations
- products without combinations
cover image of product that appears on overview pages is determined by configuration of product.
products with combinations
During configuration process of combinations you point the "default" combination of product: https://i.imgur.com/4uirJgP.png. You can also associate combination with selected images uploaded to product.
If you associated images with default combination:
- the cover image of product on overview pages is a default combination image (default - is a first one)
if product's default combination is not associated with any image:
- the cover image of product on overwiew pages is a "main image" of product
products without comniations
The cover image of products on overview pages is a "main image" of product. Always.
